- $50 deductible per individual; $150 per family
- Deductible waived for preventative services
- $1,000 annual maximum per individual
- No coverage available for orthodontic services
- Preventative services 80% / 100%
- Basic services 60% / 80%
- Major services 30% / 50%
- Eligibility requirement: 1st of the month following 90 days of service (most sites)
| Life Insurance- Provided at no cost to employee after six (6) months employment with Bartlett. Returnees’ prior experience with Bartlett is credited toward the six (6) month eligibility period
401(K) Plan- Employees man enroll on the 1st day of the month following hire date, if eligibility requirements are met

Holidays- Holidays regulated by site-specific contract language
- Administrative corporate office employees are eligible for eight paid holidays
VacationVacation time will be paid in accordance with Bartlett site-specific policies. Paid time off (PTO) should be requested and approved in advance by submitting the Vacation Request Form to your supervisor. Business needs will determine the approval. National Registry of Radiation Protection Technologists (NRRPT)- Full reimbursement of the exam fee if NRRPT exam is passed
